Personal Medication Record Athens Regional is pleased to offer you the Personal Medication Record as part of our Partners in Care program. You can use the Record to write down the medications you are currently taking, including the reason you are taking them and how often you take them. Add new medications as they are prescribed and cross out medications you have stopped taking. Carry this Record to appointments, and update information at the direction of your healthcare professional. To use, click here to download the PDF document and print out the sheet you need (English or Spanish). If preferred, cut them down to size with scissors and tape them together (printed information facing out). NOTE: This file requires the Adobe Acrobat Reader.
