Home Health- Just What the Doctor Ordered.

As part of an outstanding health care system, the mission of Athens Regional Home Health is to provide its patients with a superior value along with continuum of care–meeting the health needs of the patient outside the hospital setting.

A superior level of healthcare in the comfort and privacy of your own home.

As part of Athens Regional Medical Center, Inc., Athens Regional Home Health has access to valuable resources and skilled professionals . It will provide its patients with a service of superior value by meeting the health needs of the patient outside of the hospital setting. Caring professionals will help the patient and family understand and assume responsibility for the management of the patient's health care needs.

The transition from the hospital to the home is often times a difficult one. Individuals often need assistance with their health which cannot easily nor effectively be provided by the patients themselves, family members and friends. Athens Regional Home Health is here to smooth this transition to educate and empower each home patient to improve wellness and maximize care for oneself.

Athens Regional Home Health is a short-term home care program. Its framework of professional case management can shorten hospital stay by providing a highly personalized level of care for each patient. Doctors and nurses work together as a team to promote the independence of individuals and to help patients achieve superior level of healthcare in the comfort and privacy of their own home.

Helping the family understand and manage the patient's needs.

To qualify for home health services, a physician must first determine the need for home care and order the necessary training and treatments. With the input of the home health team, the physician develops and oversees the management of the patient's plan of care. Insurance coverage, along with home health policies and regulations, identify qualifying conditions patients must meet before home health care may be provided. A personal physician or an Athens Regional Home Health employee will assess your coverage prior to starting services.

Under the direction of your physician, skilled providers, such as Registered Nurses, Medical Social Workers, Speech Therapists, Physical Therapists and Occupational Therapists, visit the patient's home to provide necessary treatment and instruction. Here are some of the ways these team members provide services:

Skilled Nurses(RN/LPN)

  • Observe and assess the patient's condition.
  • Teach the patient, patient's family and caregiver how to manage the patient's care.
  • Provide skilled nursing procedures.
  • Supervise the Home Health Aides and student nurses about the patient's care.

Skilled nurses observe and assess the patient's condition.

Speech Therapists(ST)

  • Assess, evaluate and test the patient's prognosis and the potential for rehabilitation.
  • Provide therapy to restore communication functions affected by an illness or injury.

Medical Social Workers(MSW)

  • Assess social and emotional elements affected by illness or injury.
  • Link patients with community resources.
  • Provide counseling services.

Physical Therapists(PT)

  • Assess and evaluate the effect illness and injury has had on the patient's mobility.
  • Provide exercise to restore movement and strength.
  • Teach exercises, gait training and precautions to the patient, patient's family and caregivers.

Speech therapists evaluate and test patient's progress.

Occupational Therapist(OT)

  • Teach different ways to perform everyday tasks by regaining the function lost in illness or injury.
  • Design, make and fit self-help devices such as hand splints and special fitting utensils.
  • Adapt tools, equipment, work and living spaces to allow for self-management.


Home Health Aides(HHA)

  • Assist the patient with personal care including hygiene, exercises and movement from bed and wheelchair as instructed by skilled nurses.

Wound Therapists teach and provide ostomy, wound and incision care.

Wound Therapists(WOC)

  • Evaluate, teach and provide assistance with ostomy, wound and incision care.

Competent, compassionate, and caring professionals will assist the patient and family in understanding and assuming responsibility for the management of the patient’s health care needs.
