Call for Nominations
J.W. Fanning Humanitarian of the Year

This award was created and named in honor of Dr. J.W. Fanning who dedicated a major part of his life to community service at Athens Regional Medical Center as well as the Athens community. Through his work he directly touched the lives of thousands of individuals. Dr. Fanning attended the reception annually to honor recipients until his death in 1997.

Criteria

1. Significant contributions to ARMC through his/her time, interest, and commitment to the Medical Center and the community it serves.
2. Significant contributions, as a volunteer, in worthwhile community services that improve the health, welfare, and quality of life for area residents.
3. High personal standards in his/her life.
4. May be a physician, current or former Medical Center or Foundation Board member, or citizen of the community, but may not be a hospital employee.

Guidelines

1. The nominating committee requests that you be as thorough in your reasoning to nominate this individual as possible. This document is the basis the committee will use to determine this year's recipient.
2. You may attach any supporting documentation you have.
3. Please keep your nomination confidential.
4. Do not assume that the individual you feel should be nominated has already been nominated by someone else.
5. All nominations will be considered and the winner will be kept confidential until the award is presented.
6. Please return nomination form by Friday, March 7, 2008.

The selection committee consists of the ARHS CEO, Foundation Board Chair and Vice Chair as well as Chairs of all Development Committees. The committee uses the criteria listed above as guidelines for their selection of the winner.

Recipients receive a hand-crafted, glass sculpture with their name engraved. Their name is also engraved and placed, along with a photo, on the Wall of Honor in the Medical Center lobby.
